Inform CIC as soon as possible, but at least within one month from the date of suffering injury. The following documents must be submitted to support your claim.

• Notification of Claim
• Claimant’s/Physician’s Statement
• Employer’s Statement
• Receipts supporting medical expenses
• Police reports for assaults

Please note that you must agree to any medical examination which may be requested by the insurer
at their expense.

In case of death, the following additional documents are required:

• Death Certificate
• Original Policy Document, if Insured is deceased
• Letters of Administration
• Police Report
• Post Mortem
• Result of Coroner’s Inquest
